Getting rid of fumes - need next level help

My exhaust runs out the side and not straight back. I have plugged holes from the engine. Yet when driving with top off, especially at low speeds - from stop to 25 and back vs. continuous speed - the fumes are significant. Especially after a half hour or so - wife won't ride in the truck because she says she gets a headache. Any ideas?

Do they turn out in front or back of the rear tires? My earlier EB had header mufflers that the pipe angled back and exited in front of the rear tires and I don’t recall ever having an issue with odor. full top on or off, bikini top or not, windows up or down. Maybe mine was just a unicorn.
