Rear 3 Point Seat Belts

I installed the rear portion of my roll bar last year to provide some added protection for my kids. I now want to add three point seat belts.

Is there rules or guidelines as to the mounting location for the top point? I am mostly concerned with how high it should be. My kids are 10 and 12 but I want a height I don't have to changes as they get older or if an adult rides in the back.

To complicate things a little, I have the stock rear seat but at some point I may change that out for something that has head rests.

Any thoughts?
For those that have done this... How well do 3 point seat belts work with the stock seat. Are they hard to get at when retracted?

I just put them in my rig for the kids. I just had the largest child sit there while I guestimated the location for bungs in the cage. I had an older friend sit back there and they were okay for him. They tend to rub my kids necks, so I bought them some cushions for the straps.
