Strange leak @ Slave cylinder?

I was investigating a leak coming from the slave cylinder today. It wasn't leaking at the line or bleed valve like I thought it was. It was coming from the spring area where it's bolted to the transmission. There also appears to be a grove worn into it as well. See the picture.

Has anyone ever seen one fail in this area?
