D20 Rear Output Bearing Chewed Up

What would cause my D20 rear output shaft, outer bearing to get chewed up like this? This is the second time this particular bearing got chewed up. I recently converted to an NV3550 transmission and while I had everything out I decided to rebuild my transfer case. When I first disassembled the transfer case, the rear output shaft outer bearing was chewed up as well. I measured the endplay at .003 when I put it together. Yoke nut properly torqued. I'm afraid it's going to happen again if I don't figure out what's causing it.

Was it blu and dry upon removal , lack of oil? and was the cage bent or did you do thet to remove the rollers,, and when filling I usually pump some oil through the speedo cable hole to fill the void in the housing ..

I'm not sure if it's getting hot. I am not doing any flat towing. I don't think the race is spinning. I literally just drove around my neighborhood and surrounding area--maybe 4-5 times max over different days--after getting the NV3550 installed. I never ventured further than 5 miles. I was hearing odd noise when driving, but was hearing it when pushing the clutch pedal in as I was coming to a stop, so I assumed the noise was related to my transmission install. Maybe there is an issue there as well, I don't know.

I used Valvoline SAE 50w oil based on info I read in this forum. When filling I did not pump oil through the speedo hole.

The bearing was dry upon removal and the cage was bent and broken with some metal shaving inside the housing. A couple of the rollers fell out when I removed the bearing.

Dry bearing is the issue.......something is preventing it from oiling.....No I'm not trying to be captain obvious
