Clear Coat

The Clear Coat is slowly coming off, paint is 4 years old,

Paint is really rough where the clear came off, I thing something happen between the primer and paint.

Can it be saved or will I have to start over?

It looks like the problem is with the clearcoat, not the basecoat. The basecoat could have been sprayed too long before the clearcoat without sanding. In any case, you can probably respray just the clear, IF you can remove all of the clearcoat without damaging the basecoat. That's a very big 'if' because if the clear is bonded to the basecoat somewhere it will be hard to remove without sanding through to primer.

Unless you can do all of the work yourself it will be an expensive repair no matter what.

There's no way to save the base coat. You will need to strip all the remaining clear off with 180. Prime, sand with 500 and reapply base/clear.

The cause could have been as Steve said and the clear was applied too long after the base. My best guess would be a different brand of clear than the basecoat. It could have also been a cheap clear used.

You are starting over, chances are spending a bit more money this time to assure that you get more then 4 years out of it this time. Even the BEST case senerio here, you manage to grab a razor blade and get all the
peeling clear off,(I do mean ALL), with out harming the base. This is very unlikely, I have managed to do this on a roof but not a whole rig, even at that point I didnt trust the base so I took it down anyway and was spraying color again. If you want it to last this time do it right. a bit more time and maybe quite a bit more money , prep work makes a paint job, good prep and good material go along way.
