resurface flywheel.

You can't do it yourself without a flywheel grinding machine. It trues the flywheel and spins it and grinds it true at the same time. If your old clutch had chatter when you engage/disengage the clutch, it probably would benefit from a resurfacing.
Most engine machine shops have one of these, and it costs from 30 to 50 dollars to have one done.

Resurfacing a flywheel is not a DIY project.
Send it in and have it done. It needs to be true or it will just be another source of clutch chatter.

Not unless the clutch was severly slipped and caused heat cracks. I've reused flywheels before and just roughened up the surface a little with course sand paper just enough to get the shine off.

Are you reusing the existing clutch? If so I don't think I'd worry about it unless I thought I had an issue. If you're replacing the clutch have the flywheel resurfaced.

Depends on the condition. I reused a 302 flywheel on a 351, but I doubt it had 500 miles on it and there were no heat checks, spots, discoloration, etc. Now if it was abused in 2000 mile then it will need work. Can't see the condition, can't give an answer.
